A session-by-session guide to your SMP programme — from consultation through to your healed result and everything in between. No glossing over the parts most clinics don't talk about.
Scalp micropigmentation is a three-session treatment delivered over 5–6 weeks. Each session builds on the last. Understanding what happens in each session — and what your scalp looks like between them — makes the entire experience easier to navigate and sets realistic expectations for every stage.
Your consultation is a 45–60 minute appointment — in person at the Marlow studio or by video call. It is not a sales meeting. The purpose is to give you a clear picture of what SMP can do for your specific pattern of hair loss, and what it cannot.
During the consultation, your practitioner will:
You leave with a clear plan and no obligation to book. Most consultations result in booking, but the session exists to inform you, not close you.
Session 1 is the longest appointment of the programme. Depending on your Norwood grade and scalp surface area, expect 2.5–4 hours in the chair.
What happens: Pigment is deposited across the full treatment zone at lower than final saturation. This is intentional — the first session tests your skin's response to pigment, establishes the base density layer, and sets the hairline position. Operating at reduced density in Session 1 means any adjustments at the hairline or tonal level can be addressed in Sessions 2 and 3 without the result being over-saturated.
Your hairline position, confirmed in consultation, is marked again before treatment begins. No machine contact occurs until you have approved the position. This is not a formality — this is where mistakes get caught before they happen.
What your scalp looks like after Session 1: Reddened, noticeably darker than the healed tone, with minor swelling in the treatment zone. This is completely normal and expected. The result will look darker than the final outcome — resist the instinct to judge the result at this stage. By day 4–7 it fades significantly; by day 10–14 the healed tone is visible.
Pain level: Most clients describe the sensation as light scratching or mild discomfort — similar to the feeling of a fine-point pen drawn across the scalp. Not the searing pain associated with body tattoos. Scalp skin is thin and relatively consistent in sensitivity. The crown and temples are slightly more sensitive than the sides and back. No anaesthetic is required as standard; EMLA 5% numbing cream can be applied 45 minutes before on request.
The 7–14 days between Session 1 and Session 2 are when the epidermis fully regenerates over the deposits. Understanding what to expect during this period prevents unnecessary concern.
| Day | What you see | What's happening | Action |
|---|---|---|---|
| 0–2 | Redness, darker than expected tone, minor swelling | Normal inflammatory response to dermis penetration | Avoid washing. No gym, sun, swimming. |
| 3–5 | Surface darkening, slight dry texture | Surface pigment oxidising, minor flaking of epidermis | Gentle dry moisturise only. No scrubbing. |
| 6–8 | Visible fade — darker dots lightening | Epidermis regenerating over deposits — this is why it fades | Normal activity can resume. SPF on scalp outdoors. |
| 9–14 | True healed tone visible — lighter than Session 1 | Deposits fully encapsulated in dermis at stable depth | Scalp is ready for Session 2. Book your appointment. |
The fade between sessions is one of the aspects that surprises clients most. The result looks significantly darker immediately post-session. By the time Session 2 arrives, it will look lighter than expected. This is normal — it is why the treatment requires multiple sessions. Each session adds density that compounds with the previous.
Session 2 is where the result becomes convincingly real. Most clients report that this session is the point they first see what the final outcome will look like.
What happens: Dot density is increased across the full treatment zone. Tonal layering is introduced — darker pigment in the central crown zone, lighter and softer deposits at the hairline perimeter. This creates the optical gradient of a natural scalp with variable follicle density. The hairline edge is refined with a transitional band of lower-density deposits to prevent the hard-line appearance common in lower-quality SMP.
The Session 1 healed result is assessed before Session 2 begins. Any areas of uneven pigment uptake (sometimes seen in scar tissue zones or oily scalp areas) are addressed in Session 2 with adjusted needle depth or pigment concentration.
Session 3 is the shortest of the three — typically 1.5–2.5 hours. Its purpose is precision rather than bulk density addition.
What happens: Both practitioner and client assess the Session 2 healed result together before beginning. The session addresses:
At Session 3, long-term maintenance planning is discussed. This includes: recommended SPF use, shaving frequency, when to expect the first refresh session (typically 4–5 years), and what signs indicate it is time for a refresh.
